The unfortunate and sad struggle of Terri Schiavo
has brought the need of Living Wills and Appointed Health Care Agents center stage in our nation. I offer both the Living
Will and Appointed Health Care Agents at a cost of $5.00 each or $7.50 for both.
The Appointed Health Care Agent form
simply designates the person of your choice to make medical decisions for you if you become incapacitated. The Living Will
authorizes your agent to make choices based on your directives set forth in the event you have sufferd an injury or illness
in which doctors have determined there is no chance of recovery. It's a shame we should even need such documentation, but
it is better to be safe than sorry.
